The Whitening for Life Club™ is a unique program developed by Elison Dental Center to encourage patient oral health and to help patients’ teeth look their best at all times. One of the best parts of the program aside from having stunning white teeth is that the Club Membership is free to patients that qualify.

Promotes Consistent Cleaning
Club members visit Elison Dental Center every six months for their semi-annual cleaning. This cleaning frequency promotes healthier teeth and gums.
Professional Oversight
The teeth whitening process is monitored by health care professionals. This way, the treatment is monitored to ensure effectiveness and to protect against potential health risks.
Boost Self-Esteem
Removing unsightly and embarrassing surface stains will improve the way one feels about their smile and about themselves. An uninhibited smile contributes to a healthier lifestyle.

Free Whitening Products

Application & Enrollment
Patients interested in the Whitening for Life Club™ should call in for an appointment and fill out an enrollment provided at the office.
Terms and Conditions
The following terms and conditions apply to the Whitening for Life Club™. Please read and ask our office staff with any questions.

Getting Started
a) Membership begins upon completion of an initial exam, x-ray and cleaning.
b) Offer valid with or without insurance.
c) If you are insured, your visit may be covered 100%.
d) No charge for existing patients to join club.

Maintaining Club Membership
a) Semi-annual cleanings must be performed to maintain membership.
b) Semi-annual cleanings must be complete within 30 days of their re-call date.

Re-Enrollment
a) If semi-annual cleanings are not kept, there is a $50 re-enrollment fee.

Exclusions
a) Must be at least 16 years old to participate.
b) Dr. Elison will determine if the patient is a good candidate for whitening.
c) Offer not valid with other promotional offers.
d) Not valid for Idaho Smiles and Medicaid patients.
e) Membership is non-transferable.

Replacements
In cases of lost or damaged trays and also in the case of changes due to orthodontic or restorative work, replacement trays may be necessary. If replacements are necessary the following fees apply:
a) Replacement trays only; $12.50 single arch/ complete set $25
b) Replacement trays & molds; $25 single arch/ complete set $50
